Performs moderately complex reimbursement of claim processing work. Coordinates the proper processing of appropriate forms with assigned technical and clerical staff, transporting documents. Help implement policies and procedures related to third party client billing. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.

Under supervision of a HCS Service Coordination Supervisor, performs highly complex (senior-level) case management work. For their assigned caseload, assists persons with intellectual and developmental disabilities in developing their Person Directed Plan (PDP), which identifies services & supports needed to obtain life goals. Monitors consumers’ progress towards goals & objectives and satisfaction with services & supports available through the Home and Community-based Services program. This is completed through monthly face to face contact & frequent telephone contact with the individuals assigned, their families, and their service providers. Caseload is approximately 20 individuals.

PATH caseworker will serve as a liaison between homeless individuals and mainstream community service system. Services will be mobile in nature contacting homeless persons in the community to identify service needs to reduce homelessness by linkage to appropriate agencies within the community. Ongoing services will be provided to the homeless population identified as having a mental illness. Understands and implements trauma informed care practices. Perform health and safety duties as assigned at each Center site. Duties include community engagement with staff, consumers, and visitors, involvement in the emergency response to various incidents, routine checks of facility, grounds, and parking areas to mitigate and report health and safety hazards, safety inspections, lock and alarm checks, incident reporting, and other duties as assigned by the Health & Safety Supervisor. Use of verbal and non-verbal de-escalation techniques. Shift hours and job duties may change to support the needs of the Center.

Under the supervision of the Crisis Intervention Team Supervisor, Crisis Intervention Team Forensic Case Managers are embedded with CCPD and serve as one half of the Crisis Intervention Team. Team member is responsible for providing rapid response when dispatched with CIT officers in the community. Services to be provided include crisis assessment, intervention, and coordination of diversion from arrest and incarceration based on established processes. Team members are responsible for linking and coordinating care for eligible persons with a mental illness and/or co-occurring disorder who are being diverted from incarceration. Team member will work alongside CIT officers to facilitate services.

Performs moderately complex rehabilitation planning and instructional work. Responsible for delivery of case management and skills training for assigned caseload; coordination of consumer’s needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Performs complex professional duties related to the evaluation, individualized program planning. Under general supervision, identifies clients’ rehabilitation needs and provides the necessary instruction in teaching self-help, self-care or other skills according to the needs outlined in the client evaluations, which assist the clients in attaining their optimum levels of independence.

Contracted LCSW-Supervisor Under the Direction of the Mental Health Clinical Director. Performs routine clinical supervision of employees within MHID who are currently provisionally licensed and completing supervised practice hours in pursuit of full licensure.

Under the supervision of the Program Director for the TCOOMMI program, the Case Manager (CM) performs functions based on the TCOOMMI Program Guidelines and Processes (PGP) and contract, DHHS, JCO, and other regulatory agencies for assigned caseload. The CM will provide services to mentally ill offenders who are supervised by the parole and probation departments in order to assist them in obtaining the coping skills needed to enhance stable mental health, independence and autonomy, and community integration. The CM will provide case management functions such as linking and referring the offender to intra/inter service providers and assist them in gaining and coordinating access to necessary care and services and needed resources, conduct assessments to ensure proper services based on needs, and treatment planning.

Under the general direction of the Crisis Services Program Manager, responsible for clinical screenings, assessments, crisis intervention, and determining service eligibility based on consumer needs. Coordinates and links to needed services, provides skills training and psychosocial rehabilitation as appropriate, and monitors consumer progress with meeting treatment goals. Delivers interventions such as crisis de-escalation, case management services, treatment planning and discharge planning. Provides clinical interventions to address the need for on-going outpatient services and/or inpatient admission. This position requires travel and flexible hours. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.

Performs administrative clerical support and technical program assistance work. Duties involve: Scheduling and monitoring appointments for Adult MH Intake staff; receiving hospital referrals daily, processing Initial Contact forms completed by persons asking to be assessed for services. This includes verifying insurances, verifying if the person has submitted required documents, such as ID, Social Security card, insurance cards and proof of residence; weekly completing the deviation process for ANSAs and processing case manager emails requesting provider appointments their consumers. Processes admission paperwork and meets with consumers one on one to complete intake paperwork. Performs data entry on a daily basis. Maintains discretion with highly sensitive and confidential information as mandated.

The Team Leader’s primary goals are to build a positive relationship with participants and assist them in developing their abilities for illness self-management. The Team Leader will work with participants using a shared decision-making process to develop and modify treatment plans.

Under the supervision of IDD Transitions Support Team Supervisor, this position performs advanced (senior-level) health services or social services consultative work. Work involves assisting in the planning and execution of the Transition Support Team Services for individuals with IDD transitioning from or at risk for placement in institutions and ensuring those clients and their families receive quality services. Reviews and analyzes reports and consults on cases.

Will provide psychotherapy, skills training, and case management as authorized by HHS TRR model. Will provide therapy and rehab services using HHS approved curricula to children and adolescents 3-17 years old. Will perform intake assessments and assist in program clinical activities. These responsibilities encompass delivering crisis services within the community, conducting crisis clinical staffings, facilitating coordination and linkage with internal and external service providers based on individual needs while emphasizing the least restrictive environment. Additionally, this role involves auditing clinical files to ensure quality assurance and leading ongoing training sessions for unit staff, addressing identified training needs.

Under the supervision of the CSC Team Lead, position is responsible for the development and coordination of employment opportunities for eligible individuals in the CSC-FEP program. Performs routine vocational rehabilitation work. Work involves evaluating disabilities and conducting vocational assessments, and to provide maximum independence and employability. Duties include completion of the discovery process, job and workplace culture analysis, pre-employment training (I.e. interview skills, appearance, etc.), post-employment training (I.e. Conflict resolution, transportation training, personal issues affecting job performance, etc.), and task analysis, development of goals and objectives, job coaching, and interagency collaboration.

An Educational Resource Clinician is a Non-Physician Mental Health Professional (NPMHP) that will serve as a mental health and substance use resource for school districts located in the region served by Education Service Center (ESC) region 2. Responsible for increasing awareness and understanding of mental health and co-occurring mental health and substance use disorders. Assisting school personnel with implementing initiatives regarding these areas and ensuring local school personnel are aware of recommended best practice-based programs and other resources in the community. Facilitate trainings regarding effects of grief and trauma, substance use and abuse, intervention and prevention, and Mental Health First Aid. Must be available to deliver trainings on weekends if needed. Position will be housed at ESC 2.

Under supervision of Access Service Director and the Medical Director, provides administrative and managerial responsibilities of Walk-in Clinic, Mobile Crisis Outreach Team, and Mobile Crisis Outreach Expansion Team that includes provision of crisis services in the community, maintaining positive budget variance, clinical oversight of program and supervision of staff assigned to program.

Under supervision of the Director of IDD Services. Performs complex (journey-level) psychological work. Work involves conducting psychological evaluations.

Performs rehabilitative consumer care to include engagement into the PATH Program and linkage to other services as needed. Provides support, information, referrals, follow-up, and technical assistance to individuals served and provides general consultation to staff and community partners to assist in the successful diversion of individuals with a mental illness to appropriate treatment needs and recovery

Under supervision of the Director of Access Services, provides clinical oversight of Pathways Program, responsibilities include but are not limited to development and facilitation of treatment groups, clinical programming, and provision of crisis screenings/assessments and diagnostic evaluations. Services also include individual counseling or psychosocial rehabilitation services as directed. Responsible for providing clinical leadership and oversight of clinical work at Pathways Respite Unit as well as providing leadership and oversight of Crisis Intervention Team Services

Perform entry level highway grounds maintenance work on the Texas Department of Transportation Highway Litter Pickup contract for MHID Advance Employment Services program. Work involves completing litter pickup, debris and branch removal along major highways throughout the city and county.

Foster/Companion Care contractor provides a supervised, 24-hour arrangement in a home for intellectual developmental disability individuals who are unable to continue living independently in their own homes.
