The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Hourly Wage: $15.37

Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description

Performs collaborative work with mental health case managers, substance use providers, primary care physician offices, and other internal and external service providers to promote full integration and collaboration in clinical practice and to support the improvement of our holistic model of care.

Care Coordinators assists the consumers in obtaining needed laboratory, medical, social, financial, and other services in support of their recovery through referrals, reminders, and research of community assistance. Care Coordinators are expected to professionally and respectfully interact with internal and external service providers, request, collect and disseminate clinical information to the treatment teams and ensure coordination between external agencies that are providing interdisciplinary services. Care Coordinators follow-up on orders/referrals and problem solving as necessary to ensure services are obtained and to minimize the chances of a lapse in services or obstacle with follow-up. This position is office based and conducts most work through phone calls, though additional modes of work delivery may be assigned at the discretion of the Director of Transformation.

Care Coordinators utilize the Center's electronic health record to monitor key health indicators along with other dashboard information, such as referrals, missed appointments, crisis episodes, and more.  The Care Coordinator links individuals and families to healthcare providers, supports follow-up with internal and external providers, conducts annual care coordination assessments and routine surveys. This position requires an aptitude for researching, connecting with, and networking with a variety of local resources and disciplines as needed.  

Minimum Qualifications

Minimum of Graduation from High School or GED. Must have a valid Texas driver's license with good driving record and reliable transportation.  Preferred: At least 2 years working in a mental health setting and at least 2 years of customer service experience. Knowledge of community resources. One year direct care experience

  • Ability to work independently, organize work effectively, and communicate clearly (verbally, in writing).
  • Ability to develop productive relationships with individuals, and ability to advocate for clients when needed.
  • Knowledge of applicable rules, regulations and policies and procedures. 
  • Ability to document services in current system being used by Nueces Center for Mental   Health and Intellectual Disabilities
