The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Salary: "Please note that the job posting rate for this position is $22.98, the salary offered will follow the starting salary guidelines which are typically made at or near the minimum of the salary range. However, a salary may be negotiated above the minimum range depending on applicable years of experience and qualifications."

General Description
Performs psychosocial rehabilitation planning and instructional work. Responsible for delivery of rehabilitative training and case management services for assigned caseload; coordination of consumer's needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Seeks to reduce jail recidivism rates for people with serious emotional illnesses and high risk for recidivism who frequently cycle thorough the criminal justice system, local emergency and hospital service. Will provide on call services in order to create a bridge to treatment to increase access to mental health and substance abuse treatment. Will provide complex professional duties related to the provision of outpatient mental health services while incorporating individualized treatment planning. Under the general supervision of the Forensic ACT Team Supervisor, identifies clients' rehabilitation and service coordination needs and provides the necessary instruction in teaching self-help, self-care and skills according to the needs outlined in the client evaluations. Assists clients in attaining their optimum levels of independence and recovery. Collects, prepares, and reviews case data used to determine client eligibility for financial, medical, and/or social services. Services provided to adults of all ages with a priority population diagnosis of mental illness who exhibit serious emotional, behavioral and/or mental disorder and those at high risk for recidivism who frequent hospitals, jails, and substance use facilities. Will follow the Fidelity ACT model
Minimum Qualifications
A minimum of a bachelor's or advance degree from an accredited college or university with a major in a social, behavioral, or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and education.
Experience in social and rehabilitative services work or in teaching and counseling persons with disabilities. Valid Texas driver's license with good driving record.
Preferred: Plus, one to two years of work experience in rehabilitation services.Knowledge, Skills, and Abilities
