The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Hourly Wage: $22.06

Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description

Under supervision of the Program Supervisor, performs highly complex (senior-level) case management work.  Work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives and procedures.  May assign and/or supervise the work of others.  Responsible for monitoring incoming alerts from Nursing Facilities, scheduling and completing PASRR Evaluations (PE) to determine if MH or IDD services are required, according to HHS required timeframes. Work involves coordinating services and determining the best community option for the individual in conjunction with family and other service providers; Compliance with Joint Commission, Medicaid/Medicare regulations, and other rules; developing, scheduling, prioritizing and complying with standards set for achieving program goals; evaluating program activities; and coordinating program activities.  Works under limited supervision, with considerable latitude for the use of initiative and independent judgment. 

Participates in on-call rotation schedule to ensure timely completion of all PEs.  Will have a schedule that is flexible to the needs of consumers served and must have own transportation. May carry a caseload of approximately 20-25 individuals.

Must have reliable transportation. May use personal vehicle to conduct business.  Must have a valid driver's license at all times, liability insurance, and a good driving record.

Minimum Qualifications

Minimum: A qualified intellectual disability professional (QIDP), as defined as an employee with a minimum of a bachelor's or advance degree from an accredited college or university with a major in social, behavioral, or human service field, such as psychology, social medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, or education.

Minimum of at least one year of experience working directly with persons with intellectual disability or other developmental disabilities. Must have experience in case management or social services work.

Preferred: Experience managing medically complex cases. Spanish Speaking. Experience working with geriatric population.

Knowledge, Skills, and Abilities

  • Knowledge of individual needs/characteristics associated with intellectual and developmental disabilities.
  • Knowledge of case management principles, objectives, standards, and methods
  • Knowledge of nursing facility services, community-based resources, and community living options.
  • Knowledge of PASRR requirements and guidelines, and of program policies and procedures.
  • Skill in assessing client needs and in developing, implementing, and monitoring service and treatment plans.
  • Ability to make appropriate referrals, linkages, and to coordinate community tours and visits.
  • Ability to advocate effectively for consumers.
  • Ability to work independently, organize work effectively, and communicate clearly (verbally, in writing).
  • Ability to develop productive relationships with individuals, family members and service planning team members that focus on consumer specific outcomes.
  • Ability to monitor program effectiveness, and to assign and/or supervise the work of others.
  • Computer literate.
  • In the course of being employed, understands and implements trauma informed care practices
