The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Salary: "Please note that the job posting rate for this position is $23.25 to $25.78, the salary offered will follow the starting salary guidelines which are typically made at or near the minimum of the salary range. However, a salary may be negotiated above the minimum range depending on applicable years of experience and qualifications."

Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description

Under supervision of the IDD Intake, Eligibility, & Continuity Program Supervisor, performs advanced (senior-level) case management work. Work may involve coordinating case management and program activities, helping to develop program goals, objectives, and procedures, and evaluating outcome measures for the program. May review the work of others. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgement.

Provides on-going services to assist persons with Intellectual and Developmental Disabilities (IDD) in developing their Person Directed Plan (PDP)/ Individual Service Plan (ISP), Community Living Options (CLO), and Transition Plan. Completes screening and assessing to identify the services and supports to be provided to address the consumer's identified needs and desired outcomes including PASRR Level 1 Screening, PASRR Evaluation (PE), Discovery, Crisis Assessments, ICAPs, and other assessments as needed. Works with individuals that may have high, complex medical needs. Reviews admissions to Nursing Facilities (NF) and completes diversion request for all individuals with IDD prior to placement in NF. Coordinates educational activities for individuals residing in NFs related to community services. Completes quarterly reporting to HHSC. The work requires aggressive intervention in the community, as well as interaction and cooperation with various departments within Nueces Center for MHID. Must be compliant with Center policies and procedures, program protocols, and requirements of other regulatory bodies. Will complete waiver enrollments, Enhanced Community Coordination activities, and Service Coordination activities, as assigned. Documents case records and prepares reports. The work requires timely completion of required documentation. Performs other related work, as assigned. Caseload does not exceed 25 individuals. Will have a flexible schedule.

Must have reliable transportation. May use personal vehicle to conduct business. Must have a valid driver's license at all times, liability insurance, and a good driving record.

Minimum Qualifications

Minimum: Graduation from an accredited four-year college or university with a major in a social, behavioral, or human service field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, or education. Must meet QIDP criteria. Must have a minimum of at least two years of experience in coordinating or providing services to individuals with IDD, including those with complex Medical needs, in the community.

Preferred: Experience with Home and Community Based Services program and enrollment processes; Knowledge of the PASRR program and services; Experience working with individuals with complex medical needs; Bilingual in English and Spanish.

Knowledge, Skills, and Abilities

  • Knowledge of individual needs/characteristics associated with Intellectual and Developmental Disabilities.
  • Knowledge of provider services and community-based resources
  • Skill in individual needs assessment and treatment plan development and discharge planning. Ability to make appropriate referrals and linkages.
  • Ability to work independently, organize work effectively, and communicate clearly (verbally, in writing).
  • Ability to develop productive relationships with individuals, family members and providers that focus on consumer specific outcomes, and to advocate for consumers.
  • Ability to monitor program effectiveness, and to assign and/or supervise the work of others.
  • Knowledge of applicable rules, regulations and policies and procedures.
  • In the course of being employed, understands and implements trauma informed care practices.
