The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description

Under supervision of the General Revenue/Community-First-Choice-Texas Home Living (GCT) supervisor, performs advanced (senior-level) case management work. Work may involve coordinating case management and program activities, helping to develop program goals, objectives, and procedures, and evaluating outcome measures for the program. May review the work of others. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgement.

Provides on-going services to assist people with Intellectual and Developmental Disabilities in developing their Person Directed Plan (PDP) and Transition/Diversion Plan. Completes screening and assessments to identify the services and supports to be provided to address the consumer's identified needs and desired outcomes. Arranges for the delivery of the identified services/supports and monitors the effectiveness of these and the need for additional or different services/supports through GR, TxHmL, and HCS programs. The work requires aggressive intervention in the community and to interact and cooperate with various internal departments within Nueces Center for Mental Health and Intellectual Disabilities. Provides continuity for consumers with IDD, which includes coordinating transition planning to ensure continuity of care and management of risk factors for individuals transitioning or diverting from an institutional setting. Completes waiver enrollments, State Supported Living Center Applications, ICF/IID admissions, and permanency plans as assigned, in accordance with Center policies and procedures, program protocols and requirements of other regulatory bodies. Assists with monthly and quarterly reporting to HHSC. The work also requires timely completion of required documentation. Performs other related work, as assigned. Caseload does not exceed 30 individuals.

Must have reliable transportation. May use personal vehicle to conduct business. Must have a valid driver's license at all times, liability insurance, and a good driving record.

Minimum Qualifications

Must meet Qualified Intellectual Disability Professional criteria, including at least two years of providing Service Coordination to individuals with Intellectual and Developmental Disabilities. A qualified professional with appropriate clinical training and experience is defined as an employee with a minimum of a bachelor's or advanced degree from an accredited college or university with a major in social, behavioral, or human service field including but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and criminal justice.

Preferred: Experience with Home and Community Based Services program and enrollment processes; Knowledge of the PASRR program and services; Experience working with individuals with complex medical needs; Bilingual in English and Spanish.

Knowledge, Skills, and Abilities

• Knowledge of individual needs/characteristics associated with intellectual and developmental disabilities.
• Knowledge of provider services and community-based resources; of case management principles, objectives, standards, and methods
• Knowledge of applicable rules, regulations, and policies and procedures.
• Skill in individual needs assessment, treatment plan development, and discharge planning.
• Skill in monitoring service and treatment plans.
• Ability to make appropriate referrals and linkages.
• Ability to work independently, organize work effectively, and communicate clearly (verbally, in writing).
• Ability to develop productive relationships with individuals, family members and providers that focus on consumer specific outcomes, and to advocate for consumers.
• Ability to monitor program effectiveness, and to assign and/or supervise the work of others.
