General Description
Under the general supervision of the Intensive Services Supervisor and the guidance of the Outpatient Biopsychosocial Interventions (OBI) Lead, the Collaborative Care Case Manager (CCCM)/Qualified Intellectual Disabilities Professional (QIDP) works to provide outpatient Mental Health services for persons with Intellectual and Developmental Disabilities (IDD), their families, the community, and IDD service providers in the local service area. The CCCM completes OBI screening, Biopsychosocial assessments, and recovery planning for children and adults who have IDD or who are presumed to have IDD and who also have a co-occurring mental health condition, substance use disorder, or behavior support needs. The CCCM will provide services via a cross-systems biopsychosocial approach. Other services provided will include outreach, education, and training to community mental health partners. The CCCM will provide outpatient mental health services including collaborative care case management, skills training, and recovery planning for program participants. The CCCM will coordinate with all team members including LIDDA staff (SC, TST, CIS), provider staff, family, and other involved persons and agencies. CCCM will complete additional assessments such as CANS and ANSA, ICAP, etc as needed to establish level of need and priority for program admission and program interest list.
Minimum Qualifications
Minimum: Must meet qualifications for a Qualified Intellectual Disability Professional (QIDP) as defined by CFR 483.430 and the HHSC contract. Must have a Bachelor's or Advanced Degree from an accredited college or university with a major in social, behavioral, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and criminal justice, and at least one year of experience working with individuals with Intellectual and Developmental Disabilities. Must have reliable transportation. May use personal vehicle to conduct business. Must have a valid Texas driver's license at all times, liability insurance, and a driving record acceptable to the Center's insurance carrier.
Preferred: Two years' experience working with individuals with intellectual and developmental disabilities within the IDD service system who also have co-occurring mental health, behavioral, or substance use diagnosis. Bilingual in Spanish and English.
