Contracted PASRR Specialized Services Provider |
Under supervision of the PASRR Supervisor, consultation with the LIDDA Director and IDD Services Director, contracted agency provides Pre-Admission Screening and Resident Review (PASRR) Specialized Services to individuals with intellectual and developmental disabilities residing in nursing facilities (NF) within the Local IDD Authority's (LIDDA) service area of Nueces County. Specialized services may include independent living skills training, day habilitation, employment assistance, or supported employment. Must comply with all contract, Medicaid, HHSC, and Joint Commission standards as well as other applicable standards as they arise. |
Education, Training, and Experience (including licensure and certification) |
Agency must be Licensed as a Home and Community-based Services (HCS) agency, a Texas Home Living Services Agency (TxHmL), or a licensed Individualized Skills and Socialization (ISS) location. Persons providing specialized services under the contracted agency must meet all Health and Human Services (HHSC) or (Department of State Health Services) DSHS requirements for waiver or ISS employees/contractors, have current valid background checks, and be clear from the abuse/neglect registry. Any employee of the contractor providing transportation as part of a specialized service, must have a valid Texas Driver's License. Contractors and their employees must understand and implement trauma informed care and person-centered practices. |
Knowledge, Skills, and Abilities |
Contracted agency must have knowledge of and skills in the support and supervision needs of individuals with intellectual and developmental disabilities; of HHSC regulations; of medical and behavioral health diagnoses. Contracted agency must have the ability to provide the services delegated to the contracted agency from MHID as part of the individual's Habilitation Service Plan (HSP), the agency's contract with the LIDDA, and in compliance with HHSC's PASRR Specialized Service requirements. |

The Center is seeking to contract with a licensed HCS, TxHmL, or ISS provider to provide PASRR specialized services which may include day habilitation, independent living skills training (ILST), employment assistance, or supported employment. Day habilitation (Day Hab) may include half days or whole days. Day Hab is defined as assistance with acquiring, retaining, or improving self-help, socialization, and adaptive skills necessary to live successfully in the community and to participate in home and community life. ILST is defined as assistance provided for an individual that is consistent with the individual's HSP; is provided in the individual's nursing facility or in a community setting; includes expanded interactions, skills training activities, and programs of greater intensity or frequency beyond those a NF is required to provide; including habilitation and support activities that foster improvement of or facilitate an individual's ability to attain, learn, maintain, or improve functional living skills and other daily living activities; activities that help preserve the individual's bond with family members; activities that foster inclusion in community activities generally attended by people without disabilities; transportation to facilitate an individual's employment opportunities and participation in community activities, and between the individual's NF and a community setting; and participating in Service Planning Team (SPT) and Interdisciplinary Team (IDT) meetings. Employment assistance is assistance to an individual in locating paid, individualized, competitive employment in the community, including helping the individual identify employment preferences, job skills, work requirements and conditions; and identifying prospective employers offering employment compatible with the individual's identified preferences, skills, and work requirements and conditions. Supported Employment is provided to an individual who has paid, individualized, competitive employment in the community to help the individual sustain that employment. It includes individualized support services consistent with the HSP. Only services that are pre-authorized by MHID will be paid. Authorization must come from the PASRR Supervisor, LIDDA Director, or IDD Services Director with specific services to be provided, including identified units, frequency, and duration. Authorized services will be provided in accordance with the PASRR HSP. The contractor must ensure that documentation of service (HHSC Form 2122) for each individual is completed and submitted within 10 Center business days along with invoice for services. The contractor will ensure that credentialing standards are met, including licensure maintained. The contractor will need to submit to criminal history background check and Nurses "Abuse/Neglect" registry, or other checks as required by the Center. |
