The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Hourly/Non-Exempt: $22.20
Full-Time, 40 Hour Work Week, Paid Bi-weekly
General Description
Performs moderately complex rehabilitation planning and instructional work. Works under moderate supervision with latitude for the use of initiative and independent judgment. Coordinates and monitors delivery service. Services provided to individuals ages 3-18 in the intensive Family services and or / Youth Empowerment Services (YES) Waiver program. Services are provided within a Wraparound planning process. Perform tasks including linkage, referral, crisis management, and follow-up. A facilitator provides services that are goal- oriented activities that assist youth by locating, coordinating, and monitoring necessary care and services ensuring that they are appropriate and accessible. The facilitator coordinates and facilitates family team meetings for each youth in program and assists in developing and implementing the Plan of Care. Community-based position with flexible schedule. Works in a variety of settings.
Minimum Qualifications
A minimum of a bachelor's or advance degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention. Knowledge of community resources and case management principles. Ability to assess client needs, coordinate client services, and to communicate effectively. Comfortable presenting in front of others. Must have reliable transportation. May use personal vehicle to conduct business. Must have a valid Teas drivers license at all times, liability insurance and a good driving record.
