Under supervision of the IDD Intake, Eligibility, & Continuity Program Supervisor, performs advanced (senior-level) case management work. Work may involve coordinating case management and program activities, helping to develop program goals, objectives, and procedures, and evaluating outcome measures for the program. May review the work of others. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgement.
Provides on-going services to assist persons with Intellectual and Developmental Disabilities in developing their Person Directed Plan (PDP) and Transition/Diversion Plan. Completes screening and assessments to identify the services and supports to be provided to address the consumer's identified needs and desired outcomes. Arranges for the delivery of the identified services/supports and monitors the effectiveness of these and the need for additional or different services/supports through GR, TxHmL, and HCS programs. The work requires aggressive intervention in the community and to interact and cooperate with various internal departments within Nueces Center for Mental Health and Intellectual Disabilities.

Performs moderately complex rehabilitation planning and instructional work. Responsible for delivery of rehabilitative training for assigned caseload; coordination of consumer’s needs to maintain stability in the community and achieve independence; crisis prevention and intervention. Performs complex professional duties related to evaluation, and individualized program planning. Under general supervision, identifies clients’ rehabilitation and provides the necessary instruction in teaching self-help, self-care or other skills according to the needs outlined in the client evaluations, which assists clients in attaining their optimum levels of independence. Collects, prepares, and reviews case data used to determine client eligibility for financial, medical, and/or social services. Services provided to adults of all ages with a diagnosis of mental illness who exhibit serious emotional, behavioral and/or mental disorder.

Performs entry-level rehabilitation therapy work. Work involves providing rehabilitative services to clients with mental impairments at Pathways, a crisis respite 24-hour program. Work includes supervision of clients and their activities, observing and reporting client conditions and behaviors and interacting with clients in a crisis residential manner. Communicating with collateral staff, managing unit conditions and participating in required training programs. Works autonomously utilizing independent judgment and communicating necessary information to appropriate staff. Works under close supervision with minimal latitude for the use of initiative and independent judgment. Shifts are flex. This position is primarily 4pm – 12am. Must be flexible.

Under the supervision of IDD Transitions Support Team Supervisor, this position performs advanced (senior-level) health services or social services consultative work. Work involves assisting in the planning and execution of the Transition Support Team Services for individuals with IDD transitioning from or at risk for placement in institutions and ensuring those clients and their families receive quality services. Reviews and analyzes reports and consults on cases. Evaluates and delivers recommendations to the team. Assists with planning, developing, and administering other transition support activities and services. May provide educational or training in field of expertise. Preforms related work as assigned. Works under minimal supervision, with considerable latitude for the use of initiative and independent judgment.

An Educational Resource Clinician is a Non-Physician Mental Health Professional (NPMHP) that will serve as a mental health and substance use resource for school districts located in the region served by Education Service Center (ESC) region 2. Responsible for increasing awareness and understanding of mental health and co-occurring mental health and substance use disorders. Assisting school personnel with implementing initiatives regarding these areas and ensuring local school personnel are aware of recommended best practice-based programs and other resources in the community. Facilitate trainings regarding effects of grief and trauma, substance use and abuse, intervention and prevention, and Mental Health First Aid. Must be available to deliver trainings on weekends if needed. Position will be housed at ESC 2.

Under the supervision of the Intake Program Supervisor, the roles involve conducting screenings and assessments to determine eligibility for intakes and admission to adult mental health services. Responsibilities include completing intake procedures and conducting diagnostic evaluations for authorization under the supervision of the Intake Program Supervisor and/or other available Licensed Practitioners of the Healing Arts (LPHA). Crisis assessments and interventions are carried out promptly when necessary. The position facilitates linkage to both internal and external providers based on individuals’ needs, with a focus on discharging individuals to the least restrictive environment possible.
Certification in Cognitive Behavioral Therapy (CBT) through DSHS could be obtained. This position can offer CBT to individuals meeting the state’s level of care criteria, as well as facilitating groups, as necessary.

Under supervision of the PASRR Supervisor, consultation with the LIDDA Director and IDD Services Director, contracted agency provides Pre-Admission Screening and Resident Review (PASRR) Specialized Services to individuals with intellectual and developmental disabilities residing in nursing facilities (NF) within the Local IDD Authority’s (LIDDA) service area of Nueces County. Specialized services may include independent living skills training, day habilitation, employment assistance, or supported employment.
Must comply with all contract, Medicaid, HHSC, and Joint Commission standards as well as other applicable standards as they arise.
