Assists persons with intellectual and developmental disabilities in developing their Person Directed Plan (PDP), which identifies services & supports needed to obtain life goals. Monitors consumers’ progress towards goals & objectives and satisfaction with services & supports available through the Home and Community-based Services program. This is completed through monthly face to face contact & frequent telephone contact with the individuals assigned, their families, and their service providers. The work requires intensive coordination of private providers’ services and available community resources including Medicaid, Social Security, and other community based programs where available.


Responsible for delivering outreach services for the purpose of increasing linkage and retention in substance use, mental health, and medical services for individuals living with substance use disorders. Work involves disseminating program information to recruit program participants and presenting information to the community. Increase opportunities for substance users, including opioid users, to reduce harms related to substance use. Provides temporary and/or stop-gap coverage as assigned to assist teams within the Intellectual and Developmental Disabilities service area with the provision of HHSC contractually required services. This could include Medicaid and non-Medicaid Service Coordination activities for all IDD programs; intake screenings, assessments, interest list maintenance activities; and community living and discharge planning process activities. Based on qualifications, may provide assistance with PASRR evaluations, Habilitation Coordination, Crisis Interventions Services, Crisis Respite, and other HHSC required IDD services.


Performs complex social service work specified for consumers with a substance use diagnosis. The Licensed Chemical Dependency Counselor (LCDC) works under the general supervision of the Forensic ACT Supervisor. The primary duties of this position are to assist in the provision of treatment for conditions and problems relating to substance use disorders for the MH Adult service populations. Conducts or assists in conducting individual and group counseling sessions and instructional sessions. Facilitate and co-facilitate didactic educational presentations, group counseling, and individual counseling sessions as required per program curriculum per week.

The Licensed Chemical Dependency Counselor (LCDC) – TRA SUD Services is responsible for providing trauma-informed, evidence-based substance use disorder (SUD) treatment services to adults ages 18 and older. Works under the supervision of the TRR director. Moderate supervision, with limited latitude for the use of initiative and independent judgment. This position supports recovery-oriented care that addresses the complex needs of individuals with co-occurring mental health and substance use conditions. Services are delivered in accordance with Texas Health and Human Services Commission (HHSC) standards and agency policies, with a focus on improving client engagement, wellness, and recovery outcomes.


Under the supervision of Youth Service Director. Provides administrative and managerial responsibilities of mental health routine case management that includes maintaining positive budget variance, clinical oversite of program, and supervision of staff assigned to program. Supervises the quality and quantity of work conducted by case management staff. Responsible for auditing quality of standards of consumers’ care; assures billable services are maximized; management of unit budget; and program development. Maintains compliance of standards (JOINT COMMISSION, Medicaid, and NCMHID OPM).


Under the supervision of the PATH Program Supervisor, this role provides rehabilitative care, focuses on engaging individual into the PATH program, and linking individuals to necessary community services. This includes offering support, information, transportation, referrals, follow-up, and technical assistance to individuals served. Additionally, this role provides general consultation to staff and community partners to aid in the successful diversion of individuals experiencing homelessness and serious mental illness or co-occurring disorders to appropriate treatment and recovery services. Services are mobile in nature, involving outreach to homeless individuals in the community with the aim of mainstreaming this population into community services. The ultimate goal is to assist in the transition of individuals to obtain long-term or short-term housing solutions. This role will work under close supervision with minimal latitude for the use of initiative and independent judgment. May use personal vehicle to conduct business.


Under the general supervision of a registered Nurse, is responsible for providing nursing, mental health services and nursing/health care training to individuals in the ACT program. Responsible for delivery of intensive medical service, education, and rehabilitative training for consumers on the Assertive Community Treatment caseload; coordination of consumer’s medical-related needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Assists ACT nurse in obtaining reports and records from Med Records and performs nursing functions as appropriate. Works as a member of a team along with others on the Assertive Community Treatment program. The Educational Resource Clinician with the Behavioral Health Partnership Program (BHPP) is a mental health professional that will serve as a mental health and substance use resource for school districts located in the region served by Education Service Center (ESC) Region 2. Responsible for increasing awareness and understanding of mental health and co-occurring mental health and substance use disorders. Assisting school personnel with implementing initiatives regarding these areas and ensuring local school personnel are aware of recommended best practice-based programs and other resources in the community. Facilitate trainings regarding effects of grief and trauma, substance use and abuse, intervention and prevention, and Mental Health First Aid. Must be available to deliver trainings on weekends if needed. Position will be housed at ESC 2.


Performs moderately complex rehabilitation planning and instructional work. Responsible for delivery of rehabilitative training for assigned caseload; coordination of client’s needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Performs complex professional duties related to the evaluation, and individualized program planning. Under general supervision, identifies clients’ rehabilitation and provides the necessary instruction in teaching self-help, self-care or other skills according to the needs outlined in the client evaluations, which assists clients in attaining their optimum levels of independence. Collects, prepares, and reviews case data used to determine client eligibility for financial, medical, and/or social services. Services provided to adults of all ages with a diagnosis of mental illness who exhibit serious emotional, behavioral and/or mental disorder.


Under the supervision of the Crisis Intervention Team Supervisor, Crisis Intervention Team Forensic Case Managers are embedded with CCPD and serve as one half of the Crisis Intervention Team. Team member is responsible for providing rapid response when dispatched with CIT officers in the community. Services to be provided include crisis assessment, intervention, and coordination of diversion from arrest and incarceration based on established processes. Team members are responsible for linking and coordinating care for eligible persons with a mental illness and/or co-occurring disorder who are being diverted from incarceration. Team member will work alongside CIT officers to facilitate services. Job includes working side by side with CIT trained police officers.


Contracted Psychiatric Outpatient Services

Under oversight of the Chief Medical Officer and Mental Heath Service Director, performs highly performs advanced contracted psychiatric crisis outpatient treatment as an integral part of MHID’s Medication Clinic. Work involves assessing medical and physiological history and psychological and behavioral factors to determine the cause(s) of psychological disorders, developing plans for psychiatric treatment, and administering specialized psychiatric and medical treatments and procedures. Must comply with all contract, Medicaid, HHSC, Joint Commission, and other regulatory standards in the provision of services.


Under supervision of the IDD Intake, Eligibility, & Continuity Program Supervisor, performs advanced (senior-level) case management work. Work may involve coordinating case management and program activities, helping to develop program goals, objectives, and procedures, and evaluating outcome measures for the program. May review the work of others. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgement.


Under supervision of the IDD Service Director performs moderately complex (journey-level) managerial work administering the daily operations and activities of the Local Intellectual and Developmental Disability Authority (LIDDA). Responsible for administering the following service programs: Unit 316-IDD Intake/Admission/Enrollment, Unit 731-General Revenue & Community-First Choice (GR/CFC) Service Coordination, Unit 732-Community Living Options Information Process (CLOIP), Unit 734-Pre-admission Screening Resident Review/Texas Home Living (PASRR/TxHmL) Service Coordination, and Unit 735-Home and Community-based Services (HCS) Service Coordination. Under the general supervision of a registered Nurse, is responsible for providing nursing, mental health services and nursing/health care training to individuals in the FACT program. Responsible for delivery of intensive medical service, education, and rehabilitative training for consumers on the Forensic Assertive Community Treatment caseload; coordination of consumer’s medical-related needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Works as a member of a team along with others on the Forensic Assertive Community Treatment program. Under the supervision of IDD Transitions Support Team Supervisor, this position performs advanced (senior-level) health services or social services consultative work. Work involves assisting in the planning and execution of the Transition Support Team Services for individuals with IDD transitioning from or at risk for placement in institutions and ensuring those clients and their families receive quality services. Reviews and analyzes reports and consults on cases.


Reporting to the Access Service Director, the Crisis Services Manager assumes a pivotal role in overseeing administrative duties, managerial duties, and clinical supervision of Crisis Services programming. Additionally, the role involves coordinating with the psychiatric hotline, hospitals, jail, local law enforcement, dispatch, community partners, and Pathways respite to ensure continuity of care for consumers seeking mental health services through MHID.
