Under the general supervision of a registered Nurse, is responsible for providing nursing, mental health services and nursing/health care training to individuals in the FACT program. Responsible for delivery of intensive medical service, education, and rehabilitative training for consumers on the Forensic Assertive Community Treatment caseload; coordination of consumer’s medical-related needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Works as a member of a team along with others on the Forensic Assertive Community Treatment program. The job requires availability after hours, an automobile and extensive community travel for service provision, with a flexible schedule and hours.
Performs routine to moderately complex (Journey level) human resources administrative and technical assistance work within a human resources management department, in areas such as recruitment and selection, compensation, classification, employee relations and engagement, benefits and all backgrounds. Responsible for delivering outreach services for the purpose of increasing linkage and retention in substance use, mental health, and medical services for individuals living with substance use disorders. Work involves disseminating program information to recruit program participants and presenting information to the community. Increase opportunities for substance users, including opioid users, to reduce harms related to substance use. Under the general supervision of the director of Nursing and the Mobile Crisis Outreach team supervisor, is responsible for providing nursing, mental health services and nursing/health care training to clients and staff. Services provided to adults and children ages 3-17 with a diagnosis of mental illness who exhibit serious emotional, behavioral, or mental health disorders. Performs moderately complex work. Work involves monitoring client conditions and helping medical staff administering therapeutic programs. Works under general supervision, with limited latitude for the use of initiative and independent judgement.
Perform health and safety duties as assigned at each Center site. Duties include community engagement with staff, consumers, and visitors, involvement in the emergency response to various incidents, routine checks of facility, grounds, and parking areas to mitigate and report health and safety hazards, safety inspections, lock and alarm checks, incident reporting, and other duties as assigned by the Health & Safety Supervisor. Use of verbal and non-verbal de-escalation techniques. Shift hours and job duties may change to support the needs of the Center.
Under supervision of the Transition Support Team & IDD Crisis Services Supervisor with consultation with the IDD Services Director, contracted agency provides out-of-home crisis respite services. Services would include providing therapeutic support in a safe environment with staff on-site providing 24-hour supervision for up to 14 calendar days to an individual who is demonstrating a crisis that cannot be stabilized in a less intensive setting. Out of home crisis respite is provided in the following settings: an ICF/IID; a HCS group home; an HCS-authorized Crisis respite facility; an HHSC-authorized crisis residential facility; or a LIDDA-operated facility.
Must comply with all contract, Medicaid, HHSC, and Joint Commission standards as well as other applicable standards as they arise.
Under supervision of the PASRR Supervisor, consultation with the LIDDA Director and IDD Services Director, contracted agency provides Pre-Admission Screening and Resident Review (PASRR) Specialized Services to individuals with intellectual and developmental disabilities residing in nursing facilities (NF) within the Local IDD Authority’s (LIDDA) service area of Nueces County. Specialized services may include independent living skills training, day habilitation, employment assistance, or supported employment.

Under the supervision of the Program Director for the TCOOMMI program, the Case Manager (CM) performs moderately complex (journey-level) rehabilitative planning and instructional work. Work involves identifying client’s rehabilitative needs and providing the necessary instruction in teaching self-help or self-care and vocational skills to assist clients in attaining their optimum levels of independent living and vocational goals. CM Works under general supervision, with moderate latitude for the use of initiative and independent judgment. The CM will perform functions based on the TCOOMMI Program Guidelines and Processes (PGP) and contract, DHHS, JCO, and other regulatory agencies for assigned caseload.
Performs complex social service work specified for consumers with a substance use diagnosis. The Licensed Chemical Dependency Counselor (LCDC) works under the general supervision of the Forensic ACT Supervisor. The primary duties of this position are to assist in the provision of treatment for conditions and problems relating to substance use disorders for the MH Adult service populations. Conducts or assists in conducting individual and group counseling sessions and instructional sessions. Facilitate and co-facilitate didactic educational presentations, group counseling, and individual counseling sessions as required per program curriculum per week.
Under the supervision of Youth Service Director. Provides administrative and managerial responsibilities of mental health routine case management that includes maintaining positive budget variance, clinical oversite of program, and supervision of staff assigned to program. Supervises the quality and quantity of work conducted by case management staff. Responsible for auditing quality of standards of consumers’ care; assures billable services are maximized; management of unit budget; and program development. Maintains compliance of standards (JOINT COMMISSION, Medicaid, and NCMHID OPM).
Under the supervision of the PATH Program Supervisor, this role provides rehabilitative care, focuses on engaging individual into the PATH program, and linking individuals to necessary community services. This includes offering support, information, transportation, referrals, follow-up, and technical assistance to individuals served. Additionally, this role provides general consultation to staff and community partners to aid in the successful diversion of individuals experiencing homelessness and serious mental illness or co-occurring disorders to appropriate treatment and recovery services. Services are mobile in nature, involving outreach to homeless individuals in the community with the aim of mainstreaming this population into community services. The ultimate goal is to assist in the transition of individuals to obtain long-term or short-term housing solutions. This role will work under close supervision with minimal latitude for the use of initiative and independent judgment. May use personal vehicle to conduct business.

Assists ACT nurse in obtaining reports and records from Med Records and performs nursing functions as appropriate. Works as a member of a team along with others on the Assertive Community Treatment program. Work involves establishing goals and objectives; developing guidelines, procedures, rules, and regulations; developing schedules, priorities, and standards for achieving established goals; coordinating and evaluating program activities; and developing and evaluating budget requests. Under the general direction of the Crisis Services Program Manager, responsible for clinical screenings, assessments, follow-ups, crisis intervention, and determining service eligibility based on consumer needs. Coordinates and links to needed services as determined by screening and assessments conducted. Provides clinical interventions to address the need for on-going outpatient services and/or inpatient admission. Responsible for appropriate documentation of said clinical interventions. This position requires minimal travel and flexible hours. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.

The Educational Resource Clinician with the Behavioral Health Partnership Program (BHPP) is a mental health professional that will serve as a mental health and substance use resource for school districts located in the region served by Education Service Center (ESC) Region 2. Responsible for increasing awareness and understanding of mental health and co-occurring mental health and substance use disorders. Assisting school personnel with implementing initiatives regarding these areas and ensuring local school personnel are aware of recommended best practice-based programs and other resources in the community. Facilitate trainings regarding effects of grief and trauma, substance use and abuse, intervention and prevention, and Mental Health First Aid. Must be available to deliver trainings on weekends if needed. Position will be housed at ESC 2.
Under supervision of the IDD Intake, Eligibility, & Continuity Program Supervisor, performs advanced (senior-level) case management work. Work may involve coordinating case management and program activities, helping to develop program goals, objectives, and procedures, and evaluating outcome measures for the program. May review the work of others. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgement.
