Performs moderately complex (journey-level) computer systems analysis work. Works under general supervision, with limited latitude for the use of initiative and independent judgment. With direction from the IT Director, the employee will solve problems that are not standard and also assist staff in performing work of greater complexity. Responsible for maintaining the electronic health record, creating custom documents and reporting solutions to solve clinical and administrative issues of the Center. Works under the general direction of the IT Director with latitude for the use of initiative and independent judgement.

Assists persons with intellectual and developmental disabilities in developing their Person Directed Plan (PDP)/ Plan of Care, which identifies services & supports needed to obtain life goals. Monitors consumers’ progress towards goals, objectives & outcomes and satisfaction with services & supports available through the Community First Choice and General Revenue programs. This is completed through monthly face to face contact & frequent telephone contact with the individuals assigned, their families, and their service providers & MCOs.

Performs entry level clerical work. Primary duties are answering all incoming calls, providing pertinent information to the caller and transfers the call to the appropriate source as requested. Shows professionalism on calls and properly addresses the customer on the other end of the line. Handles emergency calls and takes the time to gather information to be able to route the call to the appropriate department. Will learn and use the paging system as needed. Gathers information on an Initial Contact form for people requesting to be assessed for services. Works under supervision with minimal latitude for the use of initiative and independent judgment. Refers problems to a lead operator or supervisor. Performs related work as assigned.

Under supervision of the IDD Service Director performs moderately complex (journey-level) managerial work administering the daily operations and activities of the Local Intellectual and Developmental Disability Authority (LIDDA). Responsible for administering the following service programs: Unit 316-IDD Intake/Admission/Enrollment, Unit 731-General Revenue & Community-First Choice (GR/CFC) Service Coordination, Unit 732-Community Living Options Information Process (CLOIP), Unit 734-Pre-admission Screening Resident Review/Texas Home Living (PASRR/TxHmL) Service Coordination, and Unit 735-Home and Community-based Services (HCS) Service Coordination. Work involves establishing program goals and objectives; developing guidelines, procedures, policies, and program manuals; developing schedules, priorities, and standards for achieving goals; coordinating and evaluating business functions, division/delegation, and department activities; developing and evaluating budget requests; and monitoring budget expenditures.

Performs complex case-management and competency restoration work: Responsible for the delivery of forensic case management and intensive rehabilitation services for eligible persons with mental illness and/of co-occurring disorders and a history of incarceration in to prevent additional incarceration.

Under the general direction of the Crisis Services Program Manager, responsible for clinical screenings, assessments, follow-ups, crisis intervention, and determining service eligibility based on consumer needs. Coordinates and links to needed services as determined by screening and assessments conducted. Provides clinical interventions to address the need for on-going outpatient services and/or inpatient admission. Responsible for appropriate documentation of said clinical interventions. This position requires minimal travel and flexible hours. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.

Provides temporary and/or stop-gap coverage as assigned to assist teams within the Intellectual and Developmental Disabilities service area with the provision of HHSC contractually required services. Under supervision of the Jail CoC Supervisor, Jail CoC case managers are responsible for activities designed to ensure an individual is provided uninterrupted services during a transition into or out of a correctional institution and that assist the individual and the individual's legally authorized representative (LAR) in identifying, accessing, and coordinating LMHA services and other appropriate services and supports in the community needed by the individual.

Under the general supervision of a registered Nurse, is responsible for providing nursing, mental health services and nursing/health care training to individuals in the FACT program. Responsible for delivery of intensive medical service, education, and rehabilitative training for consumers on the Forensic Assertive Community Treatment caseload; coordination of consumer’s medical-related needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Works as a member of a team along with others on the Forensic Assertive Community Treatment program. The job requires availability after hours, an automobile and extensive community travel for service provision, with a flexible schedule and hours.

Under the supervision of the Crisis Intervention Team Supervisor, Crisis Intervention Team Forensic Case Managers are embedded with CCPD and serve as one half of the Crisis Intervention Team. Team member is responsible for providing rapid response when dispatched with CIT officers in the community. Services to be provided include crisis assessment, intervention, and coordination of diversion from arrest and incarceration based on established processes. Team members are responsible for linking and coordinating care for eligible persons with a mental illness and/or co-occurring disorder who are being diverted from incarceration. Team member will work alongside CIT officers to facilitate services. Job includes working side by side with CIT trained police officers.

Work involves advocating and providing peer mentoring and support. Provides education to caregivers of a child or adolescent who is being served by the program. Uses personal and professional life experiences to provide consultation for staff and non-clinical skills training for families to maximize parent voice, choice, and involvement throughout the delivery of services. Assists with referrals, support and resources to help families navigate the health care system and address the unmet needs of families. Works under moderate supervision with limited latitude for the use of initiative and independent judgment.

Under supervision of the Program Supervisor, performs highly complex (senior-level) case management work. Work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives and procedures. May assign and/or supervise the work of others. Responsible for monitoring incoming alerts from Nursing Facilities, scheduling and completing PASRR Evaluations (PE) to determine if MH or IDD services are required, according to HHS required timeframes. Work involves coordinating services and determining the best community option for the individual in conjunction with family and other service providers; Compliance with Joint Commission, Medicaid/Medicare regulations, and other rules; developing, scheduling, prioritizing and complying with standards set for achieving program goals; evaluating program activities; and coordinating program activities. Works under limited supervision, with considerable latitude for the use of initiative and independent judgment.

Under the supervision of the Director for Intake Services, provides screening and assessments to determine eligibility for intakes and admission to adult mental health services. Completes intake including the biopsychosocial, diagnostic impression and ANSA. Consults and communicates with Intake Supervisor or other available LPHA in the clinical setting. Crisis assessments/interventions are completed as needed in a timely manner. Responsible for linkage to internal and external providers based on individuals needs with focus on discharge to the least restrictive environment.

Under the supervision of IDD Transitions Support Team Supervisor, this position performs advanced (senior-level) health services or social services consultative work. Work involves assisting in the planning and execution of the Transition Support Team Services for individuals with IDD transitioning from or at risk for placement in institutions and ensuring those clients and their families receive quality services. Reviews and analyzes reports and consults on cases.

Will provide psychotherapy, skills training, and case management as authorized by HHS TRR model. Will provide therapy and rehab services using HHS approved curricula to children and adolescents 3-17 years old. Will perform intake assessments and assist in program clinical activities. Works towards positive consumer outcomes by reducing symptoms and increasing level of functioning. Community based services.

Under the supervision of the Director for Intake Services, provides screening and assessments to determine eligibility for intakes and admission to adult mental health services. Completes intake including the biopsychosocial, diagnostic impression and ANSA. Consults and communicates with Intake Supervisor or other available LPHA in the clinical setting. Crisis assessments/interventions are completed as needed in a timely manner. Responsible for linkage to internal and external providers based on individuals needs with focus on discharge to the least restrictive environment. This position provides authorization for treatment through diagnostic evaluation, needs and strengths assessment and other clinical assessments to determine the lowest restrike level of care.

Performs moderately complex rehabilitation planning and instructional work. Works under moderate supervision with latitude for the use of initiative and independent judgment. Coordinates and monitors service delivery. Services provided to children and adolescents with a diagnosis of mental illness who exhibit serious emotional, behavioral or mental disorders. Responsible for delivery of rehabilitative training for assigned caseload. Services provided to individuals 3-18 years old, in the Youth Empowerment Services (YES) Waiver program.

Reporting to the Access Service Director, the Crisis Services Manager assumes a pivotal role in overseeing administrative duties, managerial duties, and clinical supervision of Crisis Services programming. Additionally, the role involves coordinating with the psychiatric hotline, hospitals, jail, local law enforcement, dispatch, community partners, and Pathways respite to ensure continuity of care for consumers seeking mental health services through MHID.

Foster/Companion Care contractor provides a supervised, 24-hour arrangement in a home for intellectual developmental disability individuals who are unable to continue living independently in their own homes.
